Pharmaceutical care plan (PCP) is a patient - centered systematic approach. Clinical pharmacist design a written format to ensure proper drug use, achieve a definite outcome and improve patient care through assuring the safety, effectiveness and cost-effectiveness.


Expected outcomes:

- Cure of a specific disease

- Alleviation or minimization of a   patient’s symptoms

- Arresting or slowing disease   progression

- Preventing a disease or symptoms


Goals of PCP:

- Identifying potential and actual drug   related problems

- Resolving actual drug-related   problems

- Preventing drug-related problems


Steps involved in creating a pharmaceutical care plan:


- Establishment of a comprehensive    patient database

- Determining the actual and potential   drug-related problems

- Development of therapeutic goals

- Identifying monitoring parameters   with end points and frequency

- Evaluation and reassessment

Assess the actual problem using SOAP format data:


S: Subjective: patient’s chief      complain or why came to hospital

O: Objective: traceable facts as vital      signs, lab results, blood pressure

A: Assessment: the medical     diagnosis on the given date

P: Plan: treatment plan including     drugs, lab orders, referrals,…     


Drug related problems:


Development of therapeutic goals:


Therapeutic goals must be developed for each problem. Goals should be definite, realistic, and measurable if possible. Therapeutic goals mostly related to:


- Approach normal physiology as     normalizing blood pressure

- Slow disease progression as in     cancer

-  Prevent adverse drug reactions

-  Consider medication costs

-  Educate the patient about drugs

   


Reassessment:

Clinical pharmacist need to review the data base and medical care plan on a regular basis, to re-prioritize patient’s problems and required intervention.
